This part looks like there might be a bug "We tried using Retrieve additional attributes from a data store--includes options to use alternate data stores and/or a failsafe mapping it first reads the table and then immediately jumps to the failsafe option". You might want to ping Harini (like in 3-4 hours from now) and ask what might be going on?
